Find the right official checklist

Open the relevant page of the city’s published Charter before preparing documents or paying fees.

Before you visit

  • PWD ID: two 1×1 photographs, one whole-body photograph, valid ID, and medical certificate from the City Health Office, as listed in the 2025 local Charter.
  • Senior Citizen ID: two 1×1 photographs and valid ID, birth certificate, or voter’s certification, as listed in the 2025 local Charter.
  • For both IDs, the Charter lists no fee, 30 minutes for forms, and seven working days for the required signatures. This separates desk handling from the release process.

What to know

PWD-ID rules: the 2021 NCDA order specifies five-year validity. The September 2026 unified-ID policy introduces lifetime validity for permanent disabilities and a phased rollout. Ask CSWDO/PDAO how existing and new cards are handled locally.

Senior-citizen and PWD discounts apply to covered goods and services under the relevant law. They are not a blanket discount on every purchase, and no general minimum purchase is stated here.

Financial assistance depends on assessment, program rules, and available funds; amounts and approval are not guaranteed.
